New procedure: Medical Referral Dr. Cals

SZV would like to inform its customers that there has been a change in the medical referral procedure to the Ophthalmology Center (Dr. Cals - Rue General de Gaulle # 6).
This change is effective immediately.

The procedure is as follows for patients requiring medical attention from the Ophthalmology Center;

• Your General Practitioner will send the medical referral for the Ophthalmology Center directly to SZV electronically (when possible). If it is not possible to send this electronically, you may be instructed by your general practitioner to bring this to SZV. This may be the only time patients will have to visit SZV.
  
  
• SZV will electronically provide the Ophthalmology Center with the required documents; medical referral & guarantee letter. Patients will no longer have to pick up a guarantee letter at SZV.

• SZV will make the patient’s appointment to the Ophthalmology Center (based on availability) and inform patients directly by telephone of their appointment date and time.
  
• Follow-up appointments, if indicated, will be made by the Ophthalmology Center and given to the patient directly at the time of their initial consult.
