Patients of Dr. Herles and Dr. van Osch who have not yet changed their family doctor can do so until November 30, 2020.

- Each family member must fill in a separate/individual form with their change request.
- FZOG insured patients: you do not have to inform SZV of your change of doctor.
- You will receive an e-mail confirmation within two (2) working days.

This means that as of October 1, 2020 more persons will only need to show a valid Sint Maarten ID or passport when visiting their health care provider.
