Important 
A change request is per person. If you would like to submit a change for your spouse/child(ren), you must submit each change request separately. 

Family doctor who will retire:
Dr. U. Tjaden will be retiring effective October 1, 2023.

 
Your confirmed doctor change request will go into effect as of October 1, 2023.
