SZV and COVID-19

On this page you will find the adjusted procedures and contact options for SZV services, as per March 17, 2020, in light of the ongoing COVID-19 pandemic. These measures have been put in place to contribute to the safety and wellbeing of our staff, customers, visitors and the community of Sint Maarten in light of the ongoing global Coronavirus (COVID-19) pandemic. We kindly ask our customers to be flexible and patient with us as we implement these changes to continue to serve in evolving conditions, with public health and safety at the forefront of all decisions.

Adjusted online and phone operating hours

Please have an active e-mail address on hand when contacting SZV. Our response time may vary between 48 – 72 hours, based on the service. We kindly ask for your patience, as we continue to serve within the current limitations.

AO-CONTROL REPORT ONLINE

When do you report unfit for work at SZV?
Private sector - on the 3rd day unfit for work
Civil Servants - on the 4th day unfit for work

Employee FAQ: Can I report for AO Doctor Control if the company I work for is closed at the moment?
SZV answer: No, if the company you work for is closed for business at the moment, you cannot be on sick-leave. It is the job of the SZV control doctor to determine when you are fit for work. You can only be fit for work if you have a place of employment that is operational.

 

Procedure AO-control online:
- Do not visit the SZV office for AO control
- Report your symptoms to your Family Doctor by phone
- Contact SZV for AO Control online - CLICK HERE 
- If you are a private sector employee, you MUST provide your Yellow Card number (red number on the upper left corner of card) and an active e-mail address when reporting online 

Effective August 13, 2020:
Important changes to AO control procedure:

For SZV, it is no longer needed to visit the family doctor on the 1st or 2nd day of unfit for work, to have the SZV yellow card signed. Only when you are unfit for work on the 3rd day, you must report to SZV via the online AO procedure.


Important:
Employees are at all times responsible for reporting to their employer that they are unfit for work. If the company you work for is currently not operational, you cannot report for sick-leave control at SZV. The sick-leave control procedure is to determine if you are fit for work at your place of employment when it is operational.





EMPLOYEE RESPONSIBILITIES

The Employee is responsible to:

  • Have a Yellow card provided by the employer (Private sector)
  • Be available for the call-back assessment by the SZV control doctor
  • Have an active e-mail address to receive confirmation by SZV
  • Report to your employer that you are unfit for work and reported this to SZV

SZV reminds insured that the adjusted procedure must be adhered to in order to maintain the right to sickness benefit via your employer.





EMPLOYER INFORMATION

Employees who have reported unfit for work according to the adjusted procedures are instructed by SZV to report this to their Employer. This is the responsibility of the Employee, and this procedure remains unchanged. When the Employer submits a request for Loss of Wages to SZV, the Employer can include the confirmation by SZV provided to the Employee or can request additional information directly to SZV via info@szv.sx.

EXPIRED MEDICAL INSURANCE

Updated June 29, 2020: If your SZV medical insurance card is now expired, please take note of the following steps:


Step 1:
Contact SZV by phone, e-mail or website for your list of required documents.

Step 2: The review process will be digital and no longer by appointment. Please send required documents in one (1) e-mail and monitor your inbox and spam-folder for replies from SZV.


Step 3: SZV will inform you when your application/renewal is approved.

If you are a government employee, you may contact the Personnel department regarding your medical insurance card.

If you are receiving medical aid assistance from government, you may contact the department of Social Services by the Government Administration Building regarding your medical insurance card.





APPOINTMENTS MEDICAL INSURANCE

Updated June 29, 2020: The process to renew your medical insurance card has changed and is now digital. If approved, an appointment will be made by SZV with a date and time for your card pick-up only. Contact SZV for information on how to renew or apply for your medical insurance card.





FIRST TIME APPLICANTS MEDICAL INSURANCE

If you are applying for the first time for yourself, spouse and/or child(ren).

Step 3: Card pick-up will take place by appointment only. When your application is approved, SZV will contact you with the date and time to pick-up your card.






BABY GUARANTEE LETTER

A baby guarantee letter can be requested via e-mail to info@szv.sx. Please click here for an overview of the procedure and required documents. Your e-mail to SZV must include the following information and required documents as attachments: Full name of expected mother, Date of birth, ID/Passport number, Civil status, if applicable: End date of residence permit, If married: same details of spouse, Name of employer of expected mother and if married, name of employer of spouse, Expected delivery date. Once approved, SZV will e-mail the St. Maarten Medical Center (SMMC) your baby guarantee letter on your behalf. You will receive an email confirmation that the approval was sent/emailed to SMMC.






PRESCRIPTIONS AND PARAMEDICAL REFERRALS


STAMP AUTHORIZATIONS

Patients do not come to SZV for their stamp.
The treating physician will send the prescription to the necessary pharmacies or provider (such as Medicosmetics) via email, with the patient or with a companion if the patient is experiencing flu-like symptoms. If a prescription needs to be stamped, the pharmacy will email us for approval. SZV will respond digitally to the pharmacy.





AUTHORIZATIONS (STRIP / LANCET / AVBZ)

Patients do not come to SZV for their approvals / stamps.
The treating physician will write a prescription for the medication, supplements or medical aids and provide this to the necessary provider (pharmacy, Medicosmetics) who will email SZV for authorization. Approvals will be given for 1 month.





PARAMEDICAL REFERRALS

Physical therapy, speech therapy, dietician etc.
This procedure does not change. Referrals for any paramedical healthcare provider should be submitted electronically as usual by the treating physician and will be processed accordingly.





MEDICAL REFERRALS ABROAD


STATUS REFERRALS ABROAD

As per March 17, 2020

• SZV patients who are currently abroad for medical referrals are being contacted and evaluated to return to Sint Maarten, conditions permitting, in accordance with Government protocols.

• All medical referrals abroad will be halted and delayed as a result of the continuous changes in entry restrictions and international medical resources.





EMPLOYERS PREMIUMS & DECLARATIONS


PAYMENT OF CESSANTIA, HEALTH AND SOCIAL PREMIUMS

Payments can be done online through your preferred Bank.

All inquiries related to payment (including requests for receipts of online payments) can be sent via e-mail to fingroup@szv.sx.

During this period SZV prefers payment online in order to maintain the required social distancing and elimination of public gatherings, as is instructed by Government officials.





EMPLOYEE MUTATION FORM

As per April 6, 2020: Employee Mutations cannot be submitted via drop-off box, but can only be submitted online via the Employer PortalClick here to go to the Employer Portal page.

Employers who do not have an account for the Employer Portal must register for an account in order to submit their Employee mutations. You can register for an account by clicking here.

DECLARATION AND OBJECTION

As per April 2, 2020: Declarations and objections can be scanned and sent via e-mail to fingroup@szv.sx.
March 2020 declarations and payments are to be submitted no later than May 4, 2020 and can be done via the Employer Portal.

PAYMENT AGREEMENTS

All requests or inquiries regarding payment agreements can be sent via email to fingroup@szv.sx.





PAYMENT DEADLINES

The deadline for Social and Health Insurance premium payment is the 15th of each month.
March 2020 declarations and payments are to be submitted no later than May 4, 2020 and can be done via the Employer Portal. 

The deadline for Cessantia payment is June 30th, 2020.

Employer’s compliance is essential to ensure optimal health of our insured.

BANK LETTER

FOR OPENING OF NEW BANK ACCOUNT (e.g. WINDWARD ISLAND BANK, SCOTIA/REPUBLIC BANK)

Your bank can assist you with this process by contacting SZV:

  • Request the account manager contact the pension department at 546-6782
  • Pension department will send a scanned copy of the letter to the bank
  • Original letter will be delivered hereafter to the bank

The Pension department is unable to (do) issue or send Life Certificate / Attestatie de Vita at this moment.

DELIVERY OF (ORIGINAL) DOCUMENTS

As per April 2, 2020: In adherence to government regulations for public health and safety, it is no longer possible to make use of our drop-off box for original documents. Please make a scan copy of your documents and e-mail this to info@szv.sx. You are required to hold on to your original documents until further notice.





CHANGE OF INFORMATION

ADDRESS, BANK ACCOUNT, MARITAL STATUS

 

  • Please use the mutation form, to inform SZV of relevant changes and e-mail this to benefits@szv.sx. 
  • Submit copy of the document(s) relevant to the mutation (e.g. utility bill in your name, bank book/statement showing bank account and your name, document confirming marriage or divorce).




FUNERAL GRANT

Local:  Guarantee letter for Emerald Funeral Home or Royal Funeral Home & Crematory

  • DO NOT COME TO SZV
  • Contact the funeral home of your choice.
  • The funeral home will contact SZV to finalize the request for the guarantee letter.
